She was the second daughter for Theodore Roosevelt and the first by his second wife, Edith. Ethel was found to be practical and a natural leader from an early age.

A 4 de Abril de 1913, casa-se com Richard Derby, um cirurgião. Durante a Primeira Guerra Mundial, a senhora Derby serviu como enfermeira em França, no American Ambulance Hospital, ajudando o seu marido. Ethel married surgeon Richard Derby in April 1913, had four children, and refused to settle into a retiring domestic life. At the outbreak of World War I in 1914 she immediately joined her husband in medical service in France—well before any of her brothers entered the war.

In 1918, her youngest brother Quentin died, followed shortly afterward by Theodore. It was to Ethel that the family again turned for quiet advice, for comfort and for the balance that every family needs.
