D'Alesandro was born in Baltimore, to Annunciata (née Lombardi) and Thomas J. D'Alesandro Jr. He was the oldest of six children, of whom his youngest sister Nancy Pelosi would later become prominent in her own right. He attended Loyola College in Baltimore and studied law at the University of Maryland School of Law.
His son, Thomas J. D'Alesandro, III, followed in his father's footsteps to become mayor of Baltimore from 1967 to 1971. Shortly before his death in August, 1987, Thomas J. D'Alesandro, Jr. had the pleasure of attending the swearing-in ceremony in Washington, D.C. for his daughter, Nancy Pelosi , a congresswoman from California.

The father and son are affectionately remembered in Baltimore as “Old Tommy” and “Young Tommy.” The occasion was marked in her native city by the renaming of the 200 block of Albemarle St., in front of the D’Alesandro home, as Via Nancy D’Alesandro Pelosi Way.

D'Alesandro was born in Baltimore on July 24, 1929, the son of Thomas J. D'Alesandro, Jr. (q.v.) (born in Baltimore in 1903) and Annunciata Lombardi (born in Naples, Italy, in 1909). The father of Thomas J. D'Alesandro III was a congressman before becoming a three-term mayor of Baltimore (1947-59).
