  4. Today, polypharmacy, a common and important problem related to drug use, occurs subsequent to this multimorbidity in the elderly in all populations.   5. By the time they reach age 60, most Americans are taking at least one prescription medication for a chronic health issue such as high blood pressure, high cholesterol or diabetes. Regular use of multiple prescriptions ― polypharmacy ― may raise the risk of overmedication, especially in older people.

  6. Polypharmacy is associated with resulting increased hospitalizations and higher costs of care for individuals and health care systems. To reduce polypharmacy, we delineate a systematic, consultative approach to identify highest-risk medications and drug-therapy problems.

  7. The most widely accepted definition of polypharmacy is the use of 5 or more medications per day. As illustrated in earlier discussion, some comorbidities require the use of multiple medications and may be appropriate for some patients. These medications have improved health and life expectancy of older adults.
