OSTEONECROSIS: ENFERMEDAD DE FREIBERG Necrosis avascular o aséptica, idiopática o secundaria a tratamiento esteroideo prolongado, traumatismos, distintas enfermedades subyacentes, trasplante renal… Cuando afecta específicamente a las cabezas de los metatarsianos se denomina enfermedad de Freiberg.
Enfermedad de Freiberg. Es una osteocondrosis de la cabeza del 2º metatarsiano, previa al cierre de la fisis y que ocurre en la 2ª década de la vida. La causa suele ser un traumatismo repetido que origina deformidad de la cabeza del metatarsiano, hipertrofia de la diáfisis y artrosis secundaria de la articulación metatarso-falángica.

La Enfermedad de Freiberg, ocurre en mujeres jóvenes debido al uso de zapatos de tacón alto. En la radiografía veremos aplanamiento de la cabeza del 2º metatarsiano (también puede aparecer en la 3º y 4º), aumento de la radiodensidad, lesiones radiolucentes, esclerosis ensanchamiento de la articulación y fragmento ostecondral (Fig 18)

Freiberg disease is most common in adolescent athletic females (M:F = 1:5) and young women. It is the fourth most common osteochondrosis and is only osteochondrosis more common in females 13.
Patients typically present with localized forefoot plantar pain, especially on weight-bearing, with swelling and tenderness.
