L'odierna provincia fu istituita con regio decreto n. 3702 del 23 ottobre 1859, la cosiddetta Legge Rattazzi, dall'unione delle province di Alessandria, Tortona, Casale, già appartenenti alla divisione di Alessandria, Asti, Acqui e Novi, quest'ultima sottratta alla divisione di Genova.
De provincie Alessandria is gelegen in het zuiden van de Noord-Italiaanse regio Piëmont. Ze wordt omgeven door de provincies Asti, Savona, Genua, Piacenza, Pavia en Turijn. Het grootste gedeelte van de noordgrens wordt gevormd door de rivier de Po. In het zuiden van de provincie ligt de voet van de Apennijnen.
