Papa Nihil is portrayed by more than one person--In the Ghost chapters, one unknown actor plays his current self, while Justin Andrews plays his younger self as he appears in visions. Justin also plays NIhil's younger self in the music videos for Dance Macabre and Kiss The Go-Goat.

Yes, Nihil himself still could play the sax before (and after) he died, but It makes sense to me that he is in his 30s in 1969 and not his 50s, because: a) Nihil would have came of age pre-WW2 if he were 50+ and been out of his element in the late 60s as a newer band playing for a younger crowd.
