He got another leading film role in 2004 when he played the title role in the comic book adaptation Hellboy. Perlman reprised his role as Hellboy in the straight to DVD animated features Hellboy: Sword of Storms (2006) and Hellboy: Blood and Iron (2007) as well as Hellboy II: The Golden Army (2008).
